Loss runs
3–5 years from each prior carrier. PDF or Excel.
Driver list
Name, DOB, license number, hire date for each driver.
MVRs
Motor vehicle records for each driver. Recent (within 12 months).
Equipment list
Year, make, model, VIN, stated value, and lien holder (if financed) for tractors and trailers.
IFTA reports
Quarterly IFTA filings if you run across state lines. Last 4 quarters.
We may need additional documentation during the underwriting process.
